"What is Congregate Living ?"
A congregate living health facility is a residential home with 6 beds, that provides inpatient care, including the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, social and recreational services.
Services are provided for persons who need long term medical care, physically disabled persons, who may be ventilator dependent; services for persons who have a diagnosis of terminal illness, a diagnosis of a life threatening illness, or both; services for persons who are catastrophically and severely disabled. The primary need of congregate living health facility residents shall be for availability of skilled nursing care on a recurring, intermittent, extended, or continuous basis. This care is generally less intense than that provided in general acute care hospitals but more intense than that provided in skilled nursing facilities.

Cochran congregate living, Inc. is a residential care facility to manage post acute care patients with medically complex illnesses. Our individual care plans are managed by an interdisciplinary group of physicians, nurses, social workers, respiratory therapist, dietitians, physical and speech therapist.
Our facility is actually a home nestled in a residential community in Simi Valley. It has 5 private rooms, fully equipped kitchen, entertainment area and a beautifully landscaped backyard promoting outdoor therapeutic activities. Patient rooms are tastefully decorated and individualized to each resident.
